Associate Membership for New & Renewing Members
You may become a Associate Member of MOWPA in one of two ways:
Via US Mail: Fill out the Membership_Application_Form FY2012 and mail along with payment to the address indicated at the bottom of the form.
Via credit card/pay pal: Fill out and submit the form below, then select the “Pay Now” button to complete your transaction. When paying by credit card or email, you must pay for each person separately.
If you are renewing your membership by Pay Pal or Credit Card, please complete the membership form below.

- Select the submit button below; then select the “Pay Now” button to complete your transaction through Pay Pal.
